If you have completed the campaign, you have probably been told that there are currently Pestilence Outbreaks across the map that appear as purple smoke. It is up to you to cleanse these areas and gain the rewards for defeating these powerful foes.
Let's break down everything you need to know about Pestilence Outbreaks in No Rest for the Wicked, so you can get plenty of endgame items.
What Are Pestilence Outbreaks?
Pestilence Outbreaks in No Rest for the Wicked are areas filled with Pestilence. When you look at the map, the infected area will have a purple haze around it, signifying that a Pestilence Outbreak is currently happening.
When you enter a Pestilence Outbreak area, you will see a haze appear on your screen and a health bar appear at the bottom. This health bar signifies how many enemies are still alive in the area.
As you defeat enemies, you will see the health bar deplete. If you leave the area, the health bar will disappear, but the amount it depleted will remain the same, so you can come and go as you please until the Pestilence Outbreak disappears.
After you defeat enough enemies to make the bar disappear, the boss of the area will be summoned. A skull marker will appear on the map, signaling the location of the boss.
The Pestilence Outbreak will disappear once the boss is defeated, and you will get various rewards for completing it. Make sure you have a strong build going into these areas. You may want to respec your character if they aren't strong enough.
When a Pestilence Outbreak has been completed, another outbreak cannot be completed in the same area for the next 24 hours. They can, however, be completed in other areas.
Summoning a Pestilence Outbreak
Unlike other events in No Rest for the Wicked, Pestilence Outbreaks are something that you can actively choose to summon. To activate a Pestilence Outbreak, start by going to a fire in a location where you want the outbreak to appear.
Interact with the fire, and you will see a new option that says "burn." This works similarly to the game's fast travel system, where you need to sacrifice your items to gain something.
Choose items from your inventory to burn in the fire. Each item you choose to burn will increase the bar at the top of the menu screen. There are various milestones for points up to 24.
The more items you offer up, the more difficult the Pestilence Outbreak will become, but the more rewards you will get from it. As the Pestilence Outbreak becomes harder, you will get more corruptions.
Corruptions are modifiers that make the Pestilence Outbreak more challenging, giving enemies buffs and you debuffs. It is a good idea to use enchanted items to enhance your character.
Below the corruption list, you will see the rewards you will get for cleansing the area. The more difficult you make the Pestilence Outbreak, the more rewards you will get.

Pestilence Outbreak Rewards
As part of completing a Pestilence Outbreak, you will get various rewards. Most of these rewards are the same as you would get from defeating a boss. For example, you will get weapons, armor, resources, and Plague Ichor.
However, some items can only be obtained by completing Pestilence Outbreaks. These are Torn Marrow, Torn Sinew, and Torn Husk.
These are important upgrade materials used to enhance your gear and create exalted items.
Creating Exalted Items
Now that you have the upgrade materials from completing Pestilence Outbreaks, you can use them to upgrade your items and make them exalted. Exalted items are ones that have enchantment enhancements beyond the normal means.
You can create exalted items by visiting the Watcher in Sacrament. When interacting with the Watcher, you will see the option to "exalt items."
Choose this option, and you will see a list of your gear. Select an item that you want to enhance, and you will see that enhancing the enchantments on it will cost a certain amount of the Pestilence Outbreak upgrade materials.
When you enhance an item through exaltation, a Radiant Ember-looking symbol will appear on the item, and one of the enchantments will be increased. After this is done four times, the item will become exalted.
