That means that all you have left in terms of bosses in your Super Meat Boy 3D journey is none other than the notorious Dr. Fetus, someone who definitely should've been kicked out of his doctoral program before earning his degree.

Compared to every other boss in Super Meat Boy 3D, I suppose Dr. Fetus is probably the toughest. But I'd also say that if you've made it this far, the worst of the game is behind you in terms of difficulty. Those last few levels in Visceraville are more troubling than this baby in a suit.
Even so, let's get you ready for this final fight. The credits await you.

The Dr. Fetus fight is a four-phase affair, so it's probably wise to watch the above video so that you can really see what you're getting into here.
In fact, if this video works for you, feel free to skip the rest of this guide, since bosses in Super Meat Boy 3D are more about memorization and execution than anything else, really. Best of luck!
How to Beat Dr. Fetus
After flipping you the double bird, Dr. Fetus fires up two lasers that form a cross and fire from his giant robo-hands. The hands will move back and forth along the side of the roof. Your best bet here is to jump rope the lasers, leaping alternating between the top-right and bottom-left quadrants of roof space. This isn't too bad to manage.
After several jumps, the lasers will begin spinning counterclockwise while maintaining their cross/X shape. Simply follow along on the ground in the open space to survive, nothing special.
After this, the fists will slam the ground and send you to phase two.
Survive two waves of hands sweeping the floor from either direction. They do this twice. Neither are hard to dodge at all.
After this, the fists will alternate punching the ground, creating a shockwave you'll have to jump over. It's possible to jump over multiple at a time, and is in fact encouraged at certain points.
The timing here is tricky, but I'd recommend watching the video above to memorize how many shockwaves you can jump over at once.
After this, you'll be sent down lower to the third phase.
Immediately, attach to the left wall, as it's the only safe space in this arena to start. From here, you'll simply hop back and forth between the walls on either side, staying off the floor as each new grid/laser alternates between walls.
After swapping walls three times, the floor will stop being electrocuted. Jump down to the floor, and the hands will carry you up to the final phase.
Back topside, Dr. Fetus will attack you directly with a laser gun. Stand to the right side of the roof, as he always swings the laser left first. Jump rope it when it comes back to the right.
Now, a giant metal ship attaches to the back of the roof. Ride up the boost arrows on the right.
Once on top of the ship, simply sprint directly towards Dr. Fetus, leap into him, and win the fight! Woo!
Oh, surprise though. After the cutscene, you'll have one more quick escape sequence with Bandage Girl in tow. This section is super short and straightforward, though. You got this!
